Langenbeck is a surgical instrument that keeps the cheek away from the work area during surgery. In the film you can see how the cheek of a patient is being held to the side using a Langenbeck during a wisdom tooth extraction. It is important that these instruments, like all surgical instruments, are only used after being sterilized. You can watch the video called "Sterility" to learn more about the sterilization process of surgical instruments. Like every operating instrument the sterile storage and usage is very important. While the instruments and isolation drapery require absolute sterility as a must, any intra-oral procedure is not sterile by definition. Many types of bacteria and fungi reside in the oral cavity and cannot be eliminated during an intra-oral procedure. Only procedures that I perform on the face and head, that have been prepped and draped and isolated can be termed a "sterile" procedure.
