pain on filled molar one year after filling

Hello, I had a cracked molar in the back filled in 2012, and had the filling replaced by another dentist in Feb 2015. (All white plastic composite filling). Filling was not done right in Feb 2015 because bite wasn't right, and I had sensitivity, and the dentist adjusted it twice. Sensitivity didn't stop. So I went to a different dentist, and he prescribed a fluoride toothpaste which stopped sensitivity for about 2 months, but about 5 months ago it returned. I resumed fluoride toothpaste, but lately the sensitivity has turned to pain that feels almost like burning sensation only when I'm drinking or eating. It's most noticeable when drinking cold liquid and the cold liquid touches the molar. When I'm not eating or drinking, there's no pain. When I feel pain when liquid touches the molar, it lasts about 5 seconds, and it stops. What could it be? Does it necessarily indicate a need for root canal?
