Bone graft right after extraction or wait for gums to heal?

Dear everyone,

I was diagnosed of aggressive periodontitis. As a result, eight of my teeth need to be extracted due to severe bone loss. The first periodontist that I went to said she could do a bone graft at the time of the extraction using a donor bone. The other periodontist that I visited said, she will first extract my teeth then I had to wait 2 months for the wound to heal before doing bone graft. This will allow my gums to heal completely because my gums were infected and I had lost a lot of bone. What do you suggest is the best way to be done? Should I wait for the area to heal, or should the bone graft be done right away and why? What is the success rate in each case? I would love to hear your advice. Thank you so much!

Clinical decision sometimes depends on which treatment you are going to choose after extraction of your teeth. For instance, bridge, implant, removable denture. It also depends on type of bone loss you have.

In any case if you need bone graft, i would prefer to have placed immediately after the extraction for couple of reasons.
1). Once you take out teeth, you basically taking out all the factors that lead to bone loss. So, probably there is no need to wait until gums heal
2). Dentist can complete both surgical procedures in single appointment. no need to open gums again once it heals
3) Some literature also suggest that immediate bone grafting after extraction (ridge preservation or ridge augmentation) help to heal the bone quickly at the the same time goes under remodeling process and preserve the bone height and width more efficiently.
4) save the time! some time it also a concern especially when teeth are located in esthetically area or crucial for chewing. You can basically save two months.
