I’ve had a root canal treatment which failed about a year ago. The endodontist performed an apicoectomy. Everything was fine until yesterday, I started to feel the same pain as I had before the apicoectomy. It’s an intermittent throbbing pain. I also had some swelling on the tooth. Does this mean the infection is back again? Is it possible to have another apicoectomy to save this tooth? It is a first molar on the bottom left side and I don’t want to lose it. Please help! It’s very frustrating.

At this point, I would cut your losses, extract the tooth, graft, and have an implant. The root is probably fractured.
